Patient Portal

Our new Patient Portal allows patients to securely perform the following functions:

• Update your demographic data
• Respond to medical history questionnaires
• View and request appointments
• Request prescription refills
• View your medical record, including lab results, medications and medication allergies, immunizations, procedures and medical history
• View your ledger and statement
• Send and receive messages

Data accessed through our Aprima Patient Portal is secure, and all patient-identifying data is encrypted, and so it cannot be compromised. Access is available only to patients and patients' authorized responsible parties. You must have a user ID and password to use your Aprima patient portal.
